About this role

As a Home Health RN Case Manager, you will coordinate and deliver individualized care to patients in their homes. You'll work independently while collaborating with a supportive interdisciplinary team, helping patients recover, maintain independence, and achieve their health goals. Develop and manage personalized care plans, perform comprehensive assessments, provide skilled nursing care, coordinate care with physicians and other healthcare professionals, support patients and families, monitor progress, document care, and build meaningful trusting relationships with patients and their families.

Requirements

  • RN license
  • Active RN license in the state of practice (Iowa)
  • RN or BSN degree from an accredited College of Nursing
  • One year clinical experience preferred; or Home care or community health nursing experience preferred
  • Car, driver's license and adequate insurance
  • Criminal background check by Iowa Division of Criminal Investigation
  • Negative finding on Office of Inspector General's Excluded Provider List
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
