*Job Summary* We are seeking a compassionate and dedicated Hospice Certified Nursing Assistant (CNA) to join our team in the South West area of Phoenix, near Tolleson. This full-time role provides essential patient care services within a hospice setting, supporting individuals at the end of life with dignity and respect. The ideal candidate will have experience working with diverse patient populations, including those with complex medical needs, dementia, or developmental disabilities. A strong understanding of medical documentation, vital signs, and infection control is essential to deliver high-quality care in this sensitive environment.
*Duties*
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and record vital signs accurately using EMR/EHR systems and document patient care activities in compliance with HIPAA regulations
- Provide medication administration under supervision, ensuring proper aseptic techniques and safety protocols are followed
- Support patients experiencing cognitive impairments such as Alzheimer’s or dementia through specialized memory care techniques
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient’s needs
