Nurse Practitioner or Physician Associate | Providence PACE

Providence PACE is seeking a full-time Nurse Practitioner or Physician Associate to join our Seaside, Oregon team. This is an excellent opportunity for a primary care provider who values team-based care and has strong interest or experience in geriatrics and palliative care. You will join an established, interdisciplinary program that provides older adults with comprehensive whole-person care, including medical, behavioral health, therapeutic, and recreational services.

Our PACE program focuses on keeping older adults active and fostering independence. Some visits will be in outpatient primary care clinics, but many visits will be in private residences, assisted living facilities, adult care homes, skilled nursing facilities, and other community-based settings. • Full-time at 1.0 FTE
• Monday to Friday, daytime hours
• Care provided in a variety of settings, primarily community-based but also outpatient clinics
• Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatric/mental health clinicians, therapists, and other interdisciplinary team members
• Ideal candidates are resourceful, clinically confident, and able to build trust quickly with patients, families, and care teams
• One year of post-graduate clinical experience working with a geriatric population preferred
• Training and/or experience in palliative care and/or hospice valuable
