Registered Nurse, Per Diem

AlphaCare Home Health is seeking an experienced Registered Nurse (RN) to provide skilled home health services, complete OASIS assessments, and coordinate patient care in collaboration with physicians, therapists, LVNs, HHAs, and caregivers. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact in patients' homes. Responsibilities include performing admissions, resumptions of care, recertifications, and discharge visits; completing OASIS assessments and maintaining accurate clinical documentation; developing and updating individualized care plans based on physician orders; assessing patient status, monitoring progress, and communicating changes to the care team; educating patients and caregivers on medications, disease management, and home care techniques; coordinating services with physicians and interdisciplinary team members; supervising and supporting LVNs and HHAs as needed; and participating in case conferences and ongoing clinical education.
