About the Role
The Field RN provides skilled nursing care to patients in their homes on an intermittent basis, following physician orders and individualized plans of care under the direction of the Director of Patient Care Services. This role is essential to improving patient outcomes, promoting independence, and enhancing quality of life while working independently as part of a supportive team.

What You'll Be Doing
Conduct initial evaluations and regularly reassess patients' nursing needs. Meet established daily productivity standards. Develop and update individualized plans of care based on patient needs. Provide skilled nursing services requiring specialized knowledge and clinical expertise. Implement appropriate preventative and rehabilitative nursing procedures. Maintain accurate clinical and progress notes for every visit. Complete timely summaries of assigned patient care conferences. Collaborate with the healthcare team to ensure coordinated, high-quality care. What We're Looking For
A genuine passion for helping others and improving patients' quality of life. Graduate of an accredited school of professional nursing. Current RN license in the state where the position is located.
