About this role

Perma Homecare is looking for a compassionate, detail-oriented Registered Nurse to join our home care team. In this role, you will be the clinical anchor for a caseload of home care clients — completing assessments, building Plans of Care that actually reflect what a client needs, and directly supervising the STNAs and Home Health Aides in the field who bring those plans to life. You will work closely with clients, families, physicians, and PASSPORT case managers to make sure every client is safe, well-cared-for, and getting the outcomes we promise.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the State of Ohio
  • Valid Ohio driver's license
  • Current, unrestricted Registered Nurse (RN) license in the State of Ohio
  • Minimum of two (2) years of nursing experience, with at least one (1) year in home care, home health, hospice, community health, or a related setting
  • Working knowledge of Ohio Medicaid home care rules, PASSPORT program requirements, and Plan of Care development
  • Current BLS/CPR certification
  • Valid Ohio driver's license, reliable transportation, and current auto insurance meeting Ohio minimum requirements
  • Ability to pass a background check consistent with Ohio Administrative Code Rule 3701-60-09 and complete pre-employment drug screening
  • Proficiency with electronic medical records (EMR) and standard office software
  • BSN preferred
