Home Care Registered Nurse

• Conduct thorough patient assessments including vital signs, physiology evaluation, and medical history review to develop personalized care plans.
• Perform vital Quality Control & Supervisory Visits with our talented care staff.
• Collaborate closely with healthcare professionals such as physicians, therapists, social workers, and case managers to coordinate patient care and discharge planning.
• Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, patient progress notes, and treatment updates while ensuring compliance with HIPAA privacy standards.
• Educate patients and their families on disease management, medication adherence, infection control (including infectious disease care), and health promotion strategies.
• Monitor patient status regularly by evaluating vital signs, conducting diagnostic evaluations, and adjusting care plans as needed for optimal outcomes.
• Manage case management responsibilities including utilization management, discharge planning, medical records review, and managing patients as a nurse case manager in various settings such as outpatient clinics or hospice care.

*Experience*

• Proven clinical experience in hospital medicine or home health environments with a focus on geriatric care. Veteran's Administration experience is a plus!
• Experience working with diverse populations including geriatrics, Veterans, behavioral health patients, or those requiring hospice services.
• Familiarity with medical terminology, health regulation policies, and electronic health records (EHR/EHR management).
• Strong patient service skills with the ability to communicate effectively with clients and collaborate seamlessly with healthcare teams.
• Knowledge of infection control protocols and HIPAA regulations to ensure patient confidentiality and safety during all interactions.
• Ability to perform comprehensive patient assessments in outpatient settings while managing multiple priorities efficiently.
