We are a network of substance abuse clinics serving clients in the Baltimore City and County area with six locations. We are currently seeking a Registered Nurse to join us as a Health Home Care Coordinator, working collaboratively with our team to provide exceptional care for our patients. Responsibilities include coordinating care for assigned participants, ensuring implementation of treatment plans, conducting health promotion and education, providing referrals and additional supports, developing wellness and prevention initiatives, delivering health education, participating in developing initial treatment plans, assisting in setting goals for individuals with chronic diseases, collaborating with community resources, coordinating with medical providers and hospitals for admissions and discharges, tracking required assessments, screenings, and immunizations, and monitoring HIT tools and reports.
