Responsibilities
Conducts initial and ongoing patient and family assessments. Implements physician orders, administers medications, performs treatments, and documents care. Develops and revises individualized care plans based on continuous assessments. Communicates effectively, maintains confidentiality, and participates in interdisciplinary team meetings. Coordinates equipment and services, mentors new staff, and oversees clinical team workflow. Participates in quality improvement initiatives and discharge planning. Provides health education to promote patient and family safety and independence, documenting outcomes.
Qualifications And Skills
Current Registered Nurse license in Illinois and Basic Life Support (CPR) certification. Bachelor’s Degree in Nursing required; 1-2 years of relevant experience preferred. Valid Illinois driver’s license.
