As a Registered Nurse (RN) at Momentum, you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Intermediate Care Facilities (ICF). The Registered Nurse (RN) role offers you the opportunity to work in a small, close-knit community where you can build meaningful relationships with both the people we support and your fellow dedicated team members.
Registered Nurse (RN) Essential
Responsibilities
- Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
- Implement and oversee the health care plans for residents
- Administer medications and perform medical procedures, as needed
- Collaborate with a multi-disciplinary team to ensure the well-being of residents
- Maintain accurate medical records and documentation
- Offer support and guidance to: License Vocational Nurse (LVN), Direct Support Professionals (DSP)
- Participate in complex case management and engage in continuous learning opportunities
Registered Nurse (RN)
Qualifications
- Valid Registered Nurse (RN) license in the state of California
- One (1) year experience as an RN; required
- Valid and current proof of legal right to drive in California; required
- CPR certification
- Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
- Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
- Strong communication and interpersonal skills
- A genuine passion for providing quality care and support to individuals with disabilities
- Ability to work effectively in a team-oriented environment
- Willingness to adapt to the needs of the residents and facility
Facility Information
- Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
- Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
- Nursing caseload typically ranges from 6-15 residents
- Our home sizes are small, including 6-bed and 15-bed facilities
- High-acuity care is limited or prohibited by facility regulations
- Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
- Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes
