Southwestern Health Resources is looking for a highly skilled Registered Nurse – RN Care Manager. Is that you?

Work Hours: Full Time Days (8:00am-5:00pm) for 40 hrs/week (remote work allowed at manager's discretion)

What You Will Do

Telephonically engage and enroll patients in designated programs based on chronic conditions or recent events. Utilizing self-management, motivational interviewing, coaching, and applying the Case Management Standards of Practice (Assess, Plan, Facilitate and Advocate) to manage care provided for assigned patient population. 25% Assess patient's current and future needs by evaluating patient/family understanding of diagnosis and health management goals, collaborating with physicians and team members, and reviewing pertinent clinical information to identify and address barriers to care.

25% . Collaboratively develop and implement a plan of care that meets the patient's healthcare goals, promotes shared decision making, optimizes allocation of resources, and facilitates patient engagement and self-care management across the care continuum. . Revises the plan of care as needed to reflect changing needs, issues and goals and evaluates the progress of the patient at prescribed minimal intervals. .

Establishes processes to monitor patient adherence to medical plan of care and utilizes teach back methods post education. 25% Facilitate symptom management and goals of care with providers and a multidisciplinary team. Address financial, logistical, and psychosocial barriers.

Maintain proper documentation including patient assessment, notes, care plans, medication reconciliation and education. 15% Reviews and manages quality reports for potential gaps in care related to chronic disease and prevention, supporting clinicians/patients in achieving quality outcomes. 5% Focuses on prevention measures consistent with established guidelines and care process models.

What You Need

  • Associate's Degree Nursing Req
  • Bachelor's Degree Nursing Pref

Experience 3 Years Related clinical experience, to include acute care or care management, pediatric clinical or care management, or maternity clinical care management experience as applicable. Req

Pediatric, maternity clinical, or care management experience is preferred

  • Licenses and Certifications
  • RN - Registered Nurse Upon Hire Req
  • CCM - Certified Case Manager 18 Months Pref

Skills Organized, detail oriented, individual with an ability to mutli-task. Great communication skills and ability to work with minimal supervision. Utilizes critical thinking skills. Has compassion and can work in a remote environment.
