Southern Family Medicine is seeking a dependable, compassionate, and team-oriented *Licensed Practical Nurse (LPN)* to join our growing primary care practice. We are looking for someone who enjoys building relationships with patients and working in a friendly, community-focused environment.

Position Responsibilities

  • Room patients and obtain vital signs and medical histories
  • Assist providers with examinations and procedures
  • Administer medications and injections as directed
  • Perform point-of-care testing and other clinical tasks

  • Assist with phone calls, patient messages, and clinical follow-up
  • Perform medication reconciliation and assist with prescription requests
  • Maintain accurate and timely documentation in the EHR
  • Assist with referrals, prior authorizations, and patient care coordination
  • Maintain clean, organized, and well-stocked clinical areas
  • Provide compassionate, professional patient care

Qualifications

  • Current Tennessee LPN license in good standing
  • Previous outpatient/primary care experience preferred
  • Strong communication and organizational skills
  • Comfortable working independently while functioning as part of a team
  • Professional, dependable, and patient-focused
