Southern Family Medicine is seeking a dependable, compassionate, and team-oriented *Licensed Practical Nurse (LPN)* to join our growing primary care practice. We are looking for someone who enjoys building relationships with patients and working in a friendly, community-focused environment.
Position Responsibilities
- Room patients and obtain vital signs and medical histories
- Assist providers with examinations and procedures
- Administer medications and injections as directed
- Perform point-of-care testing and other clinical tasks

- Assist with phone calls, patient messages, and clinical follow-up
- Perform medication reconciliation and assist with prescription requests
- Maintain accurate and timely documentation in the EHR
- Assist with referrals, prior authorizations, and patient care coordination
- Maintain clean, organized, and well-stocked clinical areas
- Provide compassionate, professional patient care
Qualifications
- Current Tennessee LPN license in good standing
- Previous outpatient/primary care experience preferred
- Strong communication and organizational skills
- Comfortable working independently while functioning as part of a team
- Professional, dependable, and patient-focused
