Clinical Services Coordinator (RN)- Home Health Infusion FT Days

Clinical Services Coordinator (RN) for Home Health Infusion. Conducts bedside infusion therapy education for patients and designated caregivers. Supports clinical staff in implementation of care plans through coordination of services, interdisciplinary collaboration, and strategies that promote patient safety. Collaborates with internal and external stakeholders to facilitate seamless transitions of care. Participates in business development and marketing initiatives. Serves as a clinical resource for referral sources, patients, caregivers, and staff regarding home infusion therapies, services, and care coordination. Provides oversight of clinical services in compliance with accepted standards of patient care.
