About this role

Atrium Health Transition Clinic is seeking a Nurse Navigator to support outreach, care coordination, and scheduling for its Transition Clinic. This is a fully remote, virtual role. The Nurse Navigator will be responsible for receiving and reviewing referrals following recent patient discharges, determining appropriate next steps, and scheduling patients with a provider or connecting them to the appropriate care team as needed.

As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Using evidence-based practice, clinical decision making, compassion, and skilled communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Requirements

  • RN
  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • BSN Required
  • 3 years of nursing experience preferred
  • Prior nurse navigation experience preferred
