Assist with Caregiver
Education
Reinforce care instructions, assist with caregiver training, and collaborate with the RN on nursing tasks and other responsibilities authorized within the LPN's scope of practice.
Monitor and Communicate Changes: Observe and document changes in client conditions, communicate concerns promptly to the RN, and assist with coordinating follow-up care as appropriate.
Support Medication-Related Needs: Assist with medication list reviews, identify and report potential medication concerns or discrepancies, document observations, and provide medication-related education or assistance as permitted by applicable regulations, agency policies, and the LPN's scope of practice.
Maintain Clinical Documentation and Compliance: Keep accurate records and assist with documentation reviews, state compliance requirements, and Missouri Department of Health and Senior Services (DHSS) survey readiness.
Collaborate and Improve: Work closely with the RN and care team, identify opportunities to improve care delivery, and contribute ideas that strengthen client outcomes and caregiver support.
Support Client and Family Engagement: Participate in care consultations and discussions about Home Instead services as appropriate to the role.
