You'll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion. What You'll Do Lead Patient-Centered Hospice Care • Conduct comprehensive patient and family assessments • Develop and manage individualized, comfort-focused care plans • Deliver skilled nursing care with a focus on pain and symptom management Serve as the Care Coordinator • Act as case manager, coordinating care across the interdisciplinary team • Collaborate with physicians, social workers, chaplains, and aides • Monitor patient condition and adjust care plans to maintain comfort Support Patients & Families • Provide education, emotional support, and guidance throughout the journey • Help families navigate complex decisions with empathy and clarity • Build meaningful, trusting relationships in deeply personal moments Drive Quality & Compliance • Maintain accurate, timely documentation • Participate in IDT meetings and care planning • Support quality and compliance standards What You Bring • Active RN license (or compact eligibility) • Graduate of an accredited nursing program • Minimum 1 year of clinical nursing experience • Strong clinical judgment and compassionate care approach • Ability to manage a caseload independently • Excellent communication skills in sensitive situations • Basic EMR proficiency • Current CPR certification • Valid driver's license and reliable transportation What Sets You Apart • Experience in hospice, palliative care, or home health • Strong pain and symptom management skills • Case management and interdisciplinary care experience • Ability to quickly build trust with patients and families • Experience with Homecare Homebase (HCHB) What to Expect • Field-based care in homes, facilities, and community settings • High autonomy with strong team support • A role centered on dignity, comfort, and quality of life • A fast-paced, emotionally meaningful environment Benefits That Support You Health & Wellness Medical, Dental, and Vision coverage Pharmacy benefits Virtual care and mental health support Flexible Spending Accounts (FSA) and Health Savings Account (HSA) Supplemental health and life insurance Financial & Protection 401(k) with company match Employee referral program Prepaid legal services Identity theft protection Work-Life Balance & Perks Generous paid time off Pet insurance Tuition and continuing education reimbursement All employment decisions are made without regard to race, color, religion, sex, gender identity or expression, sexual orientation, national origin, age, disability, veteran status, or any other protected characteristic.
