You’ll work with physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care experiences and services, improve population health, reduce cost, and achieve collaborative practices that exemplify Main Line Health System’s commitment to patient centered care, community engagement, and population health. Use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive plan of care which includes health promotion, disease prevention, and disease management activities. Position: Registered Nurse (RN) - Care Coordination Shift: Per Diem, Varied Shifts; mainly weekends, some weekdays to cover. 16 hours/month plus one weekend per month and one holiday per year. Experience: 1. Three years of recent clinical experience required. 2. Leadership experience preferred. Education: 1. Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire 2. Master’s Degree preferred. Licensures/ Certifications: Active PA RN license required
