Are you a nurse who believes that every person deserves to live their final chapter with dignity, comfort, and peace? We are looking for a compassionate RN Case Manager to join our Tucson team. In this role, you aren’t just managing a clinical caseload; you are a lifeline for patients and their families during life’s most sacred transition.

What You’ll Do

Provide comprehensive nursing care and coordination for a dedicated group of patients in the Tucson area. You will visit patients at their home or the facility in which they live.

Work closely with an interdisciplinary team (Doctor, Social Workers, Chaplains, Aides) to address the physical, emotional, and spiritual needs of your patients.

Educate and support family members as they navigate the end-of-life process.

What You Bring

A current Arizona RN license.

A heart for end-of-life care and a "people-first" mentality.

Strong organizational skills and the ability to thrive in a mobile, community-based setting.

Experience in some type of care-at-home service line, or a hunger to learn and excel in this area.

Bilingual (Spanish) a plus

HCHB EMR a plus.
