About this role
We are seeking a highly organized and detail-oriented Licensed Practical Nurse (LPN) to join our Home Health team for a full-time office-based position.

Overview
The LPN will function in a role similar to an Intake Coordinator, managing patient records, coordinating admissions, and ensuring all required documentation is complete, accurate, and compliant. Clinical knowledge will be key in supporting and streamlining internal processes.

Responsibilities
Manage and maintain patient charts and records (electronic and/or physical)
Ensure all required documentation is complete prior to admission
Track and follow up on missing documentation
Assist in organizing and maintaining compliance-ready patient files
Support clinical team with administrative tasks related to patient care coordination

Requirements
Active Licensed Practical Nurse (LPN) license in the state of Florida
Previous experience in Home Health or intake coordination preferred
Strong organizational, multitasking, and communication skills
Experience with EMR systems is a plus
Ability to work in a fast-paced office environment
Bilingual (Spanish and English)
