Overview

At Community First Solutions, our nursing team takes a resident-first approach—ensuring wellness plans are guided by each individual’s needs and goals. As a Staff Nurse LPN/RN, you’ll work side-by-side with our caregivers, providing direct bedside care, administering medications and treatments, and collaborating with physicians and the interdisciplinary team to support residents’ health and well-being. You’ll also guide and mentor caregivers, ensuring a safe, supportive, and person-centered environment. Your leadership at the bedside helps set the tone for quality, compassion, and teamwork—making Community First Solutions a place where both residents and staff thrive.

Schedules: Full Time, 7:00am - 7:00pm, 36 hours per week, every other weekend and holiday rotation

Pay: Starting at $29.00/hour, based on date of state certification.

Responsibilities

Collaborate with physicians and other interdisciplinary team members to develop, implement and manage resident-driven care plans Administer medications and treatments as ordered by Medical Director or Resident physician Provide direct leadership, supervision, and support to RA’s and STNA’s Document pertinent information following established clinical policies and procedures Effectively communicates with residents, family members, and co-workers Protects the health and safety of residents

Qualifications

Must have an active license in the state of Ohio with authorization to pass medications Ability to work 12-hour shifts Current Instructor-Led CPR Certification (or willing to obtain upon hire) Previous experience in long-term care/geriatrics preferred. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care A positive attitude and passion for working with the elderly Flexible schedule with the ability to work every 3rd weekend and holidays. This direct care position requires COVID-19 vaccination to protect our team and our clients.
