Empath Home Health , a member of Empath Health is seeking a compassionate Home Health RN Registered Nurse to work with our Home Health team to deliver individualized care to patients in their homes throughout the Labelle, Lehigh Acres, and Fort Myers area. Locations : Home health visits in Labelle, Lehigh Acres, Fort Myers, and surrounding areas.

Schedule

M-F 40 hours with weekend/holiday on-call rotations required.

What You’ll Do

Perform comprehensive patient assessments, including OASIS assessments at required intervals Develop, initiate, and revise individualized plans of care as patient needs evolve Deliver skilled nursing services and treatments as outlined in the plan of care Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies Collaborate with the interdisciplinary team to ensure seamless, holistic care Why Join Empath Health? What You’ll Need

Graduate of an approved nursing program with an active Florida RN license Prefers BSN or MSN degree Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred Strong assessment, critical thinking, and patient education skills Willingness to participate in weekend and holiday on-call rotations Unified in empathy, we serve our communities through extraordinary Full Life Care for All.
