Job Summary We are seeking a dedicated and compassionate Registered Nurse for our Home Health department. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. The Registered Nurse will work collaboratively with a multidisciplinary team to develop and implement individualized care plans. Responsibilities • Conduct comprehensive patient assessments, including physical examinations and vital signs monitoring. • Administer medications and treatments as prescribed, including IV infusion and injections. • Provide wound care, catheter care, and ostomy care as needed. • Assist with activities of daily living (ADLs) and support patients with mobility challenges using equipment such as Hoyer lifts. • Educate patients and families on health management, medication administration, and home care techniques. • Document patient care accurately in electronic medical records (EMR) systems, ensuring compliance with HIPAA regulations. • Collaborate with physicians and other healthcare professionals to coordinate patient care effectively. • Participate in discharge planning and case management for patients transitioning from hospital to home care. • Provide support for patients with chronic conditions, including pain management and acute pain management strategies. • Engage in ongoing professional development to stay current with best practices in nursing care. Experience Candidates should possess the following experience or skills: • Familiarity with EMR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. • Experience in various nursing specialties including critical care, geriatrics, pediatrics, internal medicine, hospice & palliative medicine, and emergency medicine. • Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding, and NCQA standards. • Proficiency in patient assessment techniques and diagnostic evaluations. • Experience working in diverse settings such as hospitals (including Level I & II trauma centers), nursing homes, assisted living facilities, home health agencies, or outpatient clinics. • Skills in specialized treatments, such as botulinum toxin treatment or aesthetic laser treatment, are a plus. • Previous experience working with individuals with developmental disabilities or memory care is beneficial.
