Duties
Provide advanced nursing care to patients in a variety of healthcare settings.
Perform comprehensive assessments and develop individualized treatment plans.
Collaborate with physicians and other healthcare professionals to provide holistic patient care.
Administer medications and treatments as prescribed.
Monitor and evaluate patient responses to interventions and adjust treatment plans as necessary.
Educate patients and their families on health promotion, disease prevention, and self-care strategies.
Utilize electronic medical record systems, such as eClinicalWorks, to document patient encounters and maintain accurate records.
Provide health coaching and counseling to support patients in achieving their healthcare goals.
Participate in interdisciplinary team meetings to discuss patient progress and coordinate care.

Experience
