Advanced Registered Nurse Practitioner

Duties

• Provide advanced nursing care to patients in a variety of healthcare settings.
• Perform comprehensive assessments and develop individualized treatment plans.
• Collaborate with physicians and other healthcare professionals to provide holistic patient care.
• Administer medications and treatments as prescribed.
• Monitor and evaluate patient responses to interventions and adjust treatment plans as necessary.
• Educate patients and their families on health promotion, disease prevention, and self-care strategies.
• Utilize electronic medical record systems, such as eClinicalWorks, to document patient encounters and maintain accurate records.
• Provide health coaching and counseling to support patients in achieving their healthcare goals.
• Participate in interdisciplinary team meetings to discuss patient progress and coordinate care.
• Assist with discharge planning and ensure smooth transitions of care.

• Quality Patient Care to close Medical Measure GAPS.

Experience

• Minimum of [3] years of experience as an Advanced Practice Registered Nurse (APRN).
• Experience in [Primary Care, Internal Medicine, Cardiac Care, DME,Chronic Care Management , Laboratory, is preferred.
