Job Title: Registered Nurse (RN) Case Manager – Home Health | Where Your Experience Makes the Difference Company: Home Health Care, Inc. What You'll Do Manage a home health caseload throughout St. Paul, with on-call and weekend visits within approximately 35 miles of your home. Complete OASIS assessments and individualized plans of care. Deliver skilled nursing care and patient education. Collaborate with physicians and interdisciplinary team members. Help patients safely recover and remain independent at home. Maintain accurate, timely clinical documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong clinical judgment and critical thinking skills. Excellent communication and organizational abilities. Reliable transportation for local travel.
