Review of RN's OASIS Assessments to Ensure Completion and Accuracy Record Case Information and Complete All Necessary Forms with Accuracy Assist with Case Management including Draft Service Plans, Review Case Progress, Determine Case Closure Facilitate Multiple care aspects including Case Coordination, Information Sharing, Discharge Ensure Requests for Visits are Filled Out and Sent to Appropriate Staff Follow-up on Patient Cases Oversee All Patients to Ensure Care is Initiated in an Efficient and Timely Manner Review Clinical Records to Ensure Services Have Been Provided in Accordance with the Patient's POC Ensure Physician Orders are Signed. Communicate with the PCP and/or Insurance Co. Regarding Changes in Condition, the Need for New/Modified Orders, etc., Meet with Direct Care Staff and Administrative Staff to Communicate any Follow-ups or Concerns in Regard to Patients Hold and Document Case Conferences with other Members of the Healthcare Team Assist with Verifying Notes for Billing Verify Appropriate Documentation Resides in Patient's Clinical Record Develop Effective Working Relationships with Clinical Team Throughout Lifespan of Case Management Process

Knowledge, skills, experience required

Must Have a Valid RN License (State of Florida) Minimum of 3 Years Home Health Experience OASIS Home Health Background Possess a Good Level of Computer Literacy Experience in Microsoft Office (Word, Excel, Outlook, and PowerPoint) Knowledge of Medicare Guidelines Authorization Experience Level 2 Background screening- More information can be found at: https://info.flclearinghouse.com/
