Licensed Practical Nurse (LPN) – Home Health – Full-time / Part-time / Per diem ICK-Assurance Home Health Care, Inc. | Fitchburg, MA | 978-342-0081

Pay: $30 per medication administration visit (MAV) / $35 per skilled nursing visit. Per-visit pay includes the visit, its documentation, and travel between visits. Your hourly equivalent depends on how visits are clustered. Mileage between visits is reimbursed, and overtime is paid as required by law.

ICK-Assurance Home Health Care is a Medicare-certified home health agency and MassHealth provider caring for patients in their homes in Worcester and Middlesex Counties.

What you will do

• Make skilled nursing visits following the practitioner-signed plan of care, under the supervision of an RN Case Manager.
• Do a focused assessment at each visit: vital signs, wounds, blood sugar, pain and response to treatment. Report anything outside parameters to the RN right away.
• Give ordered medications and treatments, such as injections, wound care, ostomy and catheter care, within your verified skills.
• Check medications in the home against the medication profile and report any differences the same day.
• Reinforce teaching started by the RN, and check the home for safety at every visit.
• Document each visit in our EMR within 24 hours, and clock in and out with EVV.
• This role does not include start-of-care visits, OASIS assessments or aide supervision. Those are done by the RN.

What you need

• Active, unrestricted Massachusetts LPN license, or a compact license valid in MA
• Current BLS for Healthcare Providers (AHA or equivalent)
• Valid driver’s license, reliable transportation and auto insurance. The job requires driving between homes.
• Home health, long-term care or sub-acute experience preferred
• Ability to use a smartphone or tablet for documentation and EVV
