Summary
The Hospice Registered Nurse (RN) is responsible for providing skilled nursing care to hospice patients in their place of residence and coordinating patient care with the Interdisciplinary Team (IDT), physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process.
This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently in a hospice field setting while collaborating with the interdisciplinary care team.
Qualifications
- Registered Nurse (RN), licensed per state requirements
- Hospice experience preferred
- Home Care Home Base (HCHB) experience preferred
- Strong clinical assessment, communication, and documentation skills
- Experience working within an interdisciplinary healthcare team
- Knowledge of hospice regulations, documentation, and care planning preferred
- Reliable transportation, valid driver’s license, and current auto liability insurance
- Ability to read, write, and communicate effectively
