Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County. We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly. Summary of essential job functions: Provide skilled nursing care. Provide case management services. Conduct initial assessments of new patients. Travel to patients wherever they reside: Home, nursing facility etc.
