Licensed Practical Nurse

• Provide one-on-one nursing care in the patient’s home according to the established Plan of Care.
• Monitor the patient’s condition and report changes or concerns to the supervising RN/Director of Nursing.
• Administer medications and treatments as ordered.
• Perform skilled nursing procedures within the LPN scope of practice and according to the patient’s needs.
• Monitor vital signs and overall health status.
• Assist with activities of daily living and personal care as appropriate.
• Maintain patient safety and infection-control precautions.
• Complete accurate nursing documentation for each shift.
• Communicate professionally with the patient, family, nursing supervisor, and other members of the care team.
• Follow all agency policies, physician orders, and applicable state and regulatory requirements.

Qualifications

• Current and active *Florida LPN license*.
• Current *CPR/BLS certification*.
• Current Level II background screening or ability to complete required screening.
• Previous private-duty, home health, pediatric, or skilled nursing experience preferred.
• Ability to work independently in a home setting.
• Dependable transportation and reliable attendance.
• Strong communication and documentation skills.
• Compassionate, professional, and patient-centered approach to care.
