What you will do

Provide physician orders per agency and regulatory requirements and within the limits of practice. The plan of care and re-evaluation occurs no less often than every 60 days.

Comprehensive documentation appropriately reflects patient care from admission to discharge utilizing all Oasis assessment tools.

Serves as a case manager directing multidisciplinary patients' plans of care

Participates in the agency’s quality assurance program

Provides written documentation of assessment and care delivery in a professional manner according to policy and within the designated period.

Ideal Candidate

Active NYS Registered Nurse Licensure/Registration

Active NYS Driver’ License, CPR/BLS Card, and COVID vaccinated

One year of Registered Nurse experience is preferred, however, new graduates are welcome to apply

Previous homecare experience and/or acute care or rehab experience is a plus

Oasis Knowledge is a plus

Work Location: In person
