- Provide expert guidance on symptom management, including pain control, shortness of breath, nausea, and other comfort measures aligned with hospice care principles.
- Collaborate with interdisciplinary teams to coordinate urgent interventions such as medication adjustments or referrals to emergency services when necessary.
- Document all patient interactions accurately within electronic health record (EHR) systems, ensuring compliance with HIPAA and other regulatory standards.
- Educate patients and families about hospice services, medication administration, and advanced care planning to support informed decision-making.
- Triage incoming calls from patients, families, and healthcare providers, prioritizing cases based on severity and complexity.
- Maintain up-to-date knowledge of hospice protocols, symptom management techniques—including IV infusion and aseptic technique—and relevant legal considerations such as Workers' Compensation law.
Requirements
- Valid nursing license (RN or LPN) with current certification in Basic Life Support (BLS).
- Proven experience in hospice & palliative medicine or related fields such as critical care, emergency medicine, or home health nursing.
- Familiarity with EMR/EHR systems
- Strong assessment skills with knowledge of physiology, anatomy, and diagnostic evaluation techniques.
- Ability to work independently during weekend shifts while maintaining excellent communication with team members.
- Experience working with individuals with disabilities or in long-term care settings such as assisted living or nursing homes is highly desirable.
