Responsibilities
Conduct thorough patient assessments via phone or in-person during weekend shifts, utilizing clinical judgment to determine urgency and appropriate response.
Provide expert guidance on symptom management, including pain control, shortness of breath, nausea, and other comfort measures aligned with hospice care principles.
Collaborate with interdisciplinary teams to coordinate urgent interventions such as medication adjustments or referrals to emergency services when necessary.
Document all patient interactions accurately within electronic health record (EHR) systems, ensuring compliance with HIPAA and other regulatory standards.
Educate patients and families about hospice services, medication administration, and advanced care planning to support informed decision-making.
Triage incoming calls from patients, families, and healthcare providers, prioritizing cases based on severity and complexity.
Maintain up-to-date knowledge of hospice protocols, symptom management techniques—including IV infusion and aseptic technique—and relevant legal considerations such as Workers' Compensation law.
