About this role

Registered Nurse, Case Manager. The Registered Nurse serves as the case manager for assigned patients and as the teacher for patient's caregiver(s). The RN case manager utilizes the nursing process and the six phases of the Alive CARE visit to provide safe, end of life care to the terminally ill patient/family unit.

The primary responsibility of the RN case manager is to coordinate and provide hospice care for home care patients through the development, initiation and updating of a plan of care in collaboration with patient and family. RN case managers provide direct patient care, oversee the interdisciplinary team and teach patients and families regarding the disease process, symptom management and offer support and strength during difficult times. Care delivery takes place in the patient's home or in any other facility where the patient may reside or receive care.
