Your work family includes doctors, nurses, on-call nurses, NP's, triage nurses, admission nurses, LPN's, social workers, CNA's, chaplains, and volunteers, just to name a few...the only thing missing is YOU! Description: As an Admissions Nurse, you will be an active member of the Interdisciplinary Team for providing skilled nursing assessments on an as-needed basis consistent with Tapestry Hospice policies & procedures. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician to establish an initial plan of care while acting as a liaison between the various team members during the admission process.
