LPN - All Shifts

We are seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our dedicated team at Kings Harbor Multicare Center, a premier skilled nursing and rehabilitation facility in the Pelham Gardens neighborhood of the Bronx. As an LPN with our team, you will have the opportunity to make a meaningful difference in the lives of our residents, who come from diverse backgrounds and require a range of care levels.

Responsibilities

Provide high-quality, person-centered nursing care to our residents, promoting their independence, dignity, and well-being

Assess, develop, and implement individualized care plans that address the unique needs and goals of each resident

Administer medications, treatments, and therapies as prescribed by our medical staff

Collaborate with our interdisciplinary healthcare team to ensure seamless coordination of care and services

Document resident care and progress accurately and timely

Educate and support residents, families, and caregivers on resident care and needs
