West Valley Adult Day Health Care is a Community Based Adult Services(CBAS) program that looks after the well-being of adults and provides an array of mental and social services, designed to maximize the independence of those who are experiencing significant problems with their health and mental abilities. By enrolling in the program, participants can improve or maintain their physical mobility and function.

West Valley Adult Day Health Care Center (ADHC) in Woodland Hills, CA is looking for RN(s) to work on a Full-time or Part Time basis.

• Responsible for managing nursing and clinical support.

• Provides leadership for nursing and clinical support functions.

• Works collaboratively on clinical operations with the Administrative Leadership Team.

• Serves as team leader.

• Develops and implements quality, cost-effective, efficient systems.

*Responsibilities*

• Responsible for ensuring teaching, training and competencies for center staff as needed.

• Maintains acceptable standards of nursing care, based on CBAS standards.

• Works collaboratively with the staff in planning and implementing medical services.

• Assist with monitoring of quality and compliance with all nursing standards (including, but not limited to, development of audits for nursing documentation and facility audits).

• Instructs patient and/or family on follow-up care.

• Documents activities (e.g. medication, nursing care plans, etc.).

• Communicates, coordinates and consults with members of the health team while performing nursing responsibilities.

• Responds to all medical emergencies and will take the lead in management of the situation as indicated.

• Performs other related duties as assigned.
