Position Summary
Smoky Mountain Home Health and Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to join our team as a road nurse/staff nurse. This position primarily involves providing direct patient carer through home visits within our service area (Sullivan County) and surrounding areas, delivering skilled nursing services to patients in their homes or residential facilities.

The road nurse/staff nurse is responsible for performing patient assessments, providing skilled nursing care, implementing physician-ordered treatments, and collaborating with the interdisciplinary care team to ensure high-quality, patient-centered care. This role plays an essential part in supporting patients and families while promoting comfort, maintaining dignity, and improving overall quality of life. Due to the nature of home health and hospice services and the patient population we serve, participation in an on-call rotation is required for this position to ensure continuity of care and timely response to patient needs outside of regular business hours.
Key Responsibilities
- Conduct comprehensive patient assessments and assist in the development and implementation of individualized plans of care in accordance with physician orders and agency policies.
- Provide skilled nursing services including medication administration, wound care, symptom management, and monitoring of patient conditions in accordance with physician’s orders.
- Travel to patient homes and facilities within the designated service area (Sullivan County) and surrounding areas to provide care.
- Educate patients and caregivers on disease process, medications, treatment plans, and home care needs as part of care planning.
- Communicate patient status changes to physicians and appropriate members of the interdisciplinary team.
- Complete accurate and timely clinical documentation (within 24 hours) in compliance with agency policy and regulatory requirements.
- Collaborate with the interdisciplinary care team including case managers, therapists, social workers, and chaplains.
- Participate in care conferences (IDT’s) weekly and assist in care coordination as needed.
- Participate in the agency’s on-call rotation schedule, providing after-hours support as required.
- Adhere to all Medicare, state, and agency regulations governing home health and hospice care.
- Maintain patient confidentiality and ensure patient safety during all home visits.
- Must be able to lift 50lbs or greater.
Qualifications
- Current unencumbered Registered Nurse (RN) license in the State of Tennessee.
- Minimum one year of nursing experience as a licensed RN; home health or hospice experience is a plus.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently while maintaining effective communication with the care team.
- Excellent interpersonal and patient education skills.
- Must maintain a safe driving record and provide proof of current driver’s license and automobile insurance upon hire and updated as required thereafter, to comply with TN state laws.
- Current CPR certification required.
- Ability to pass a background check, drug screening, and driving record review in accordance with company policy.
- Ability to meet the physical demands of the position, including lifting, bending, and assisting patients as needed.

Due to the travel/on-call requirements associated with providing care within our service area (Sullivan County), preference may be given to applicants who currently reside within or near the community served by this role.
