Responsibilities

Experience/Qualifications

Licensed Practical Nurse (LPN) license to practice in the respective state.

Provide nursing care under supervision while ensuring patient safety and comfort.

Accurate documentation of assessments, treatments, and outcomes in EMR systems.

Administer medications per protocol and monitor for adverse reactions.

Adhere to regulatory standards and workplace safety policies at all times.

Collaborate with physicians and care teams to develop and update care plans.

Track patient progress against defined targets and KPIs; report variances.

Utilize forecasting pipelines to anticipate patient care needs and staffing demands.

Engage in daily huddles, providing timely updates on patient status and workload.
