About this role
First State Home Health & Hospice Care is seeking an experienced *Home Health Registered Nurse (RN)* to join our office team. This position is ideal for an RN who enjoys clinical documentation, quality assurance, chart review, and Plan of Care development.
This is *not a field position*. The nurse will work in the office reviewing charts, creating and updating Plans of Care, assisting with chart audits, and helping ensure compliance with Medicare home health regulations.
We are looking for someone who is highly organized, detail-oriented, and willing to learn and follow our agency's established processes and documentation standards.
Responsibilities

- Create and update Home Health Plans of Care (POCs)
- Review charts for completeness, accuracy, and compliance
- Scrub charts prior to billing and audits
- Review OASIS documentation for consistency
- Identify missing documentation and follow up with clinicians
- Assist with physician orders, recertifications, and care coordination documentation
- Support quality assurance and compliance activities
- Help prepare records for ADRs, audits, and medical review requests
- Work closely with office staff and clinical leadership
Requirements
✅ Active Michigan RN License
✅ Minimum 2 years of Home Health experience
✅ Strong knowledge of Medicare Home Health regulations
✅ Experience with OASIS documentation
✅ Experience developing and reviewing Plans of Care
✅ Strong computer and EMR skills
✅ Excellent attention to detail
✅ Ability to work independently and meet deadlines
Preferred Qualifications
- QA or chart audit experience
- Medicare ADR experience
- DeVero/Netsmart experience
- Previous Clinical Manager or Documentation Review experience
