Part-Time Home Health RN – Clinical Documentation & Plan of Care Nurse

• Create and update Home Health Plans of Care (POCs)
• Review charts for completeness, accuracy, and compliance
• Scrub charts prior to billing and audits
• Review OASIS documentation for consistency
• Identify missing documentation and follow up with clinicians
• Assist with physician orders, recertifications, and care coordination documentation
• Support quality assurance and compliance activities
• Help prepare records for ADRs, audits, and medical review requests
• Work closely with office staff and clinical leadership

Requirements

✅ Active Michigan RN License

✅ Minimum 2 years of Home Health experience

✅ Strong knowledge of Medicare Home Health regulations

✅ Experience with OASIS documentation

✅ Experience developing and reviewing Plans of Care

✅ Strong computer and EMR skills

✅ Excellent attention to detail

✅ Ability to work independently and meet deadlines

Preferred Qualifications

• QA or chart audit experience
• Medicare ADR experience
• DeVero/Netsmart experience
• Previous Clinical Manager or Documentation Review experience

Pay: $32.00 - $38.00 per hour Work Location: In person
