The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient's status, initiates plan of care, develops care plan with palliative nursing actions, administers medications and treatments, counsels patient and family, provides health care instructions, acts as Case Manager when assigned. Participates in on-call duties as defined by on-call policy.

Requirements

RN License
Current state licensure
Graduate of National League of Nursing accredited school of nursing
Minimum of 1 year experience as a professional nurse within the last 3 years OR baccalaureate degree in nursing from NLN accredited program with current RN license
Minimum of 2 years experience preferred, at least 1 year in public health, home care, or hospice nursing
Bachelor's degree in nursing
Hospice experience
On call availability
