The Resource RN Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the teams in the ambulatory setting. The nurse will perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews. The nurse will work alongside a team of RN care coordinators within the DHC.

The OHSU registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The RN’s care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics and meets the standards/expectations of a Professional Practice Model. In that model, the RN demonstrates the professional role obligations of scientist, leader, practitioner, and knowledge transferor. Professional accountability enriches the RN’s engagement as a leader in promoting an inter-professional culture of collaborative decision-making, innovation, life-long learning, and teamwork.

The qualifications for the position are the minimum requirements to be eligible for an interview. The level of required experience and expertise for each posted position is influenced by the current amount of knowledge and skills of the other nurses in the unit/departments.

The patient care coordinator role depends heavily on the RN Role Components of Scientist, Leader, Practitioner and Knowledge Transferor as defined by the OHSU RN Role Description; however, the position requires advanced critical thinking within the Leader Component to meet the goals of the clinic.

Candidate must be detail-oriented and able to work well with electronic health record.

Required Qualifications

1-2 years of nursing experience inpatient with a complex medical focus, or in ambulatory or community-based setting working with chronically ill or medically complex patients. BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date. ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.

Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire Current, unencumbered Oregon State Registered Nurse License WA license within 6 months BLS from AHA required. New hires will be enrolled and required to complete during orientation. Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

Experience in the field of surgery / GI medicine in an Ambulatory clinical setting is preferred

Length of Orientation – Experienced Nurse

External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week. Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
