About this role
Clinic RN responsible for coordinating and providing direct and indirect nursing care to participants at the PACE center and in participants' homes. Works in close coordination with the PACE Physician, clinic staff, and interdisciplinary team members to manage ongoing care for frail elderly participants in North County San Diego. Includes conducting nursing assessments, home visits, administering medications, care transitions, and end-of-life care coordination.

Requirements
- Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
- Unencumbered California Registered Nurse (RN) License
- Current Basic Life Support (BLS) / First Aid Certification required
- Original valid driver's license with proof of auto insurance
- Minimum of two years nursing experience in a clinical setting with a frail or elderly population
- Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations
- Medical clearance for communicable diseases and up-to-date immunizations
- Pre-employment drug screen, background check, and physical assessment
