Licensed Practical Nurse (LPN)

We’re looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. In this role, you’ll provide hands-on nursing care to older adults in a supportive and collaborative environment. You’ll work under the direction of the Program Director and Registered Nurse (RN) and help supervise program aides involved in participant care.

Qualifications

• Valid LPN license from the Massachusetts Board of Registration in Nursing.
• Certified by a Massachusetts-licensed physician confirming good health and freedom from communicable disease (including tuberculosis) prior to employment.
• Experience working with older adults and individuals with cognitive impairments is a plus.
• Ability to work flexible or unscheduled hours as needed.
• Strong teamwork skills and a commitment to quality care.

Responsibilities

• Monitor participants’ health and report changes to the Program Director.
• Record vital signs and weights.
• Deliver nursing care and treatments as directed by the RN.
• Assist in documenting care, including monthly nurses’ notes and health education (if delegated).
• Provide skincare and nail care to participants.
• Follow infection control practices.
• Prepare diet orders and oversee meal service based on participants’ dietary needs.
• Report concerns with food or fluid intake.
• Assist with emergency care when needed.
• Supervise program aides assisting with nursing services.

Pay: $32.00 - $38.00 per hour
