About this role
We’re looking for a compassionate and dedicated *Licensed Practical Nurse (LPN)* to join our team. In this role, you’ll provide hands-on nursing care to older adults in a supportive and collaborative environment. You’ll work under the direction of the Program Director and Registered Nurse (RN) and help supervise program aides involved in participant care.

- Valid LPN license from the Massachusetts Board of Registration in Nursing.
- Certified by a Massachusetts-licensed physician confirming good health and freedom from communicable disease (including tuberculosis) prior to employment.
- Experience working with older adults and individuals with cognitive impairments is a plus.
- Ability to work flexible or unscheduled hours as needed.
- Strong teamwork skills and a commitment to quality care.

Responsibilities

- Monitor participants’ health and report changes to the Program Director.
- Record vital signs and weights.
- Deliver nursing care and treatments as directed by the RN.
- Assist in documenting care, including monthly nurses’ notes and health education (if delegated).
- Provide skincare and nail care to participants.
- Follow infection control practices.
- Prepare diet orders and oversee meal service based on participants’ dietary needs.
- Report concerns with food or fluid intake.
- Assist with emergency care when needed.
- Supervise program aides assisting with nursing services.

Pay: $32.00 - $38.00 per hour
