What You'll Do
As a Hospice Registered Nurse Case Manager, you will
- Manage and coordinate care for hospice patients and families
- Complete assessments, develop plans of care, and coordinate interdisciplinary services
- Provide patient and family education regarding disease progression, symptom management, and end-of-life care
- Collaborate with physicians, providers, social workers, chaplains, and care teams
- Participate in on-call rotations with additional compensation
- Advocate for patient comfort, dignity, and quality of life
Qualifications
- Current Arizona Registered Nurse license
- Valid driver's license and reliable transportation
- Strong assessment and critical thinking skills
- Excellent communication and documentation abilities
- Comfortable working independently while remaining connected to a team
- Hospice experience preferred, but not required
