What You'll Do

As a Hospice Registered Nurse Case Manager, you will

• Manage and coordinate care for hospice patients and families

• Complete assessments, develop plans of care, and coordinate interdisciplinary services

• Provide patient and family education regarding disease progression, symptom management, and end-of-life care

• Collaborate with physicians, providers, social workers, chaplains, and care teams

• Participate in on-call rotations with additional compensation

• Advocate for patient comfort, dignity, and quality of life

Qualifications

• Current Arizona Registered Nurse license

• Valid driver's license and reliable transportation

• Strong assessment and critical thinking skills

• Excellent communication and documentation abilities

• Comfortable working independently while remaining connected to a team

• Hospice experience preferred, but not required
