AiLi Home Health is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes throughout the Fresno area.
This is a 1099 independent contractor (per diem) position offering a flexible schedule and competitive per-visit compensation.
Responsibilities
Read the full posting
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Routine Skilled Nursing visits.
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as ordered by the physician..
- Provide wound care, catheter care, injections, medication management, and patient education.
- Collaborate with interdisciplinary teams in case management to coordinate care effectively.
- Monitor patient progress and communicate changes to the physician and interdisciplinary team.
- Complete accurate and timely documentation..
- Educate patients and caregivers on disease management, medications, safety, and fall prevention.
- Participate in case conferences and coordinate patient care with therapists and other healthcare providers.
Qualifications
- Current California Registered Nurse (RN) License
- Home Health experience preferred
- OASIS documentation experience preferred
- Current BLS Certification
- Valid California Driver's License
- Reliable transportation and current auto insurance
- Excellent communication and organizational skills families.
- Ability to work independently and manage your own schedule
