AiLi Home Health is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes throughout the Fresno area.

This is a 1099 independent contractor (per diem) position offering a flexible schedule and competitive per-visit compensation.

Responsibilities

Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Routine Skilled Nursing visits.

Perform comprehensive patient assessments and develop individualized care plans.

Administer medications and treatments as ordered by the physician..

Provide wound care, catheter care, injections, medication management, and patient education.

Collaborate with interdisciplinary teams in case management to coordinate care effectively.

Monitor patient progress and communicate changes to the physician and interdisciplinary team.

Complete accurate and timely documentation..

Educate patients and caregivers on disease management, medications, safety, and fall prevention.

Participate in case conferences and coordinate patient care with therapists and other healthcare providers.

Qualifications

Current California Registered Nurse (RN) License

Home Health experience preferred

OASIS documentation experience preferred

Current BLS Certification

Valid California Driver's License

Reliable transportation and current auto insurance

Excellent communication and organizational skills families.

Ability to work independently and manage your own schedule
