Registered Nurse Case Manager

We are seeking an experienced Home Health Registered Nurse Case Manager for a 13-week contract opportunity serving Vermont and New Hampshire communities. This role supports home health patients through assessment, care planning, case management, patient education, and coordination with physicians, caregivers, and interdisciplinary care teams. Responsibilities include screening, evaluating, re-evaluating, and treating home health patients; developing and updating individualized care plans; coordinating patient care through interdisciplinary communication and case conferences; assessing clinical, functional, and emotional status; and documenting evaluations, treatment goals, care updates, and patient education accurately using EPIC.
