*Overview*
We are hiring a Full-Time RN to perform in-home patient assessments across our service area. This role offers a steady caseload with the independence of field work and the support of an experienced clinical team.

What You’ll Do
- Conduct in-home nursing assessments (initial evaluations, reassessments, and PRN visits)
- Evaluate patient condition including physical, cognitive, and psychosocial status
- Review medications and assess safety risks and fall risk
- Complete documentation within:
- 48 hours (initial & PRN visits)
- 5 days (reassessments)
- Collaborate with physicians and care team to ensure continuity of care
What You Need
- Active RN license (in-state)
- Minimum 1 year of nursing experience
- Home health or case management experience preferred
- Reliable transportation and valid driver’s license
- Strong time management and documentation skills
