  • Perform initial patient assessments upon admission, including comprehensive OASIS (Outcome and Assessment Information Set) data collection to support accurate reimbursement and quality reporting.
  • Review and interpret medical records, documentation, and clinical data to ensure compliance with Medicare regulations, CMS guidelines, and state healthcare policies.
  • Collaborate with interdisciplinary teams to develop personalized care plans that meet patient needs while adhering to clinical documentation standards.
  • Conduct utilization management activities by reviewing medical necessity, supporting discharge planning, and ensuring appropriate level of care.
  • Maintain proficiency in EMR/EHR systems such as Wellsky ( Kinnser), and other electronic health record platforms for accurate documentation review and data entry.
  • Stay current with healthcare regulations including HIPAA, long-term care regulations, ICD coding (ICD-9/ICD-10), CPT coding, DRG assignments, and Medicare/Medicaid policies.
  • Support clinical documentation improvement initiatives by ensuring complete and precise medical records that reflect the patient’s condition and services provided.
  • Participate in ongoing education related to acute care experience, hospital inpatient experience, long-term care regulations, case management practices, and utilization review processes.

*Requirements*

  • Valid RN license with current state licensure; experience in home health nursing preferred.
  • Demonstrated expertise in OASIS data collection and familiarity with CMS guidelines for home health agencies.
  • Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/ICD-10), CPT coding, MDS (Minimum Data Set), and DRG classifications.
  • Experience with EMR/EHR systems such as Wellsky or similar platforms; proficiency in Microsoft Office Suite is a plus.
  • Background in managed care environments or Medicare/Medicaid reimbursement processes highly desirable.
  • Knowledge of health regulation policies including HIPAA compliance and state healthcare regulations governing home health services.
  • Excellent communication skills to coordinate effectively with interdisciplinary teams and external providers.
  • Ability to review medical records thoroughly while adhering to clinical documentation standards; strong attention to detail is essential.
