No per-visit pay, no unpaid charting, and a growing patient census that keeps your schedule steady. At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture.
We're growing our San Francisco team and looking for a compassionate, organized RN Case Manager. Whether you're a seasoned home health nurse or a hospital nurse looking for a change of pace, we offer a structure that respects your license, rewards your expertise, and supports you in the field. Why SF Nurses Choose Health Link

- AI-assisted charting: Smart documentation tools on company-issued iPads that cut charting time and keep your evenings free.
- Real-time support: Our clinical leadership and intake teams are available in real time when you need orders, coordination, or a second opinion. You're never on your own.
- A manageable pace with organized scheduling and a team that respects your time.
