Registered Nurse

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge (DC) assessments
• Complete comprehensive head-to-toe assessments and develop individualized care plans
• Monitor patient condition and evaluate response to treatment
• Administer medications, injections, and treatments as prescribed
• Perform skilled nursing procedures including wound care, IV therapy, catheter care, and disease management
• Provide education to patients and caregivers on medications, disease processes, safety, and self-care
• Coordinate care with physicians and interdisciplinary team members (LVN, PT, OT, ST, MSW, HHA)
• Supervise and delegate tasks to LVNs and Home Health Aides (HHAs)
• Ensure accurate, timely, and compliant documentation per agency, state, and federal regulations (OASIS, Medicare guidelines)
• Identify and report changes in patient condition and initiate appropriate interventions
• Participate in case conferences and quality improvement activities

Qualifications

• Active California Registered Nurse (RN) license in good standing
• Current CPR/BLS certification
• Minimum 1–2 years of nursing experience (home health experience preferred)
• Experience with OASIS documentation preferred
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to travel to patient homes within assigned territory
• Strong clinical, organizational, and documentation skills

• Strong knowledge of home health regulations and Medicare guidelines
• Competency in assessment, care planning, and clinical decision-making
• Experience with wound care, IV therapy, and chronic disease management
• Excellent communication and patient education skills
• Ability to work independently and manage a flexible schedule
• Critical thinking and problem-solving abilities

Supervision

• Reports to Director of Nursing (DON) / Clinical Supervisor

Pay: $75.00 - $120.00 per hour Work Location: In person
