Role: Registered Nurse in a long-term care and post-acute rehab community.
Responsibilities
Assess residents' clinical status and develop individualized care plans. Administer medications and treatments following protocols. Coordinate with an interdisciplinary team to support resident recovery, comfort, and dignity. Monitor and document residents' progress in the EHR system. Educate residents and their families on care and health management. Mentor and support CNAs and LPNs to ensure quality care delivery.
